Job Description: Park Forest Care Center Registered Nurse (RN) 

Position Overview:

As a Registered Nurse (RN) in a long-term care nursing home, your role is to provide skilled nursing care, leadership, and coordination of care for residents. You will be responsible for assessing residents' health needs, developing and implementing care plans, administering medications and treatments, and supervising nursing staff. Your primary focus will be on delivering high-quality, person-centered care and ensuring the overall well-being of residents.

Key Responsibilities:

  1. Resident Assessment and Care Planning:

  • Conduct comprehensive assessments of residents' physical, mental, and emotional health, including medical history, vital signs, and laboratory results.

  • Collaborate with the interdisciplinary team, including physicians, nurse practitioners, and other healthcare professionals, to develop individualized care plans based on residents' needs and preferences.

  • Monitor and evaluate residents' conditions, track changes, and modify care plans as necessary.

  1. Medication Administration and Treatment:

  • Administer medications to residents as prescribed, ensuring accuracy, adherence to medication protocols, and compliance with facility policies.

  • Perform complex treatments and procedures, such as wound care, intravenous therapy, and pain management, using evidence-based practices.

  • Monitor residents' response to medications and treatments, documenting any adverse reactions or changes in condition.

  1. Nursing Care and Leadership:

  • Provide direct nursing care to residents, including assessment, planning, implementation, and evaluation of care.

  • Coordinate and supervise the activities of nursing staff, including Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), ensuring adherence to care plans and maintaining high-quality standards.

  • Ensure proper delegation and supervision of nursing tasks, while promoting a positive and collaborative work environment.

  1. Documentation and Reporting:

  • Maintain accurate and detailed documentation of nursing assessments, care plans, interventions, and resident responses.

  • Complete required reports, forms, and charts in a timely and accurate manner, adhering to facility policies and regulatory requirements.

  • Communicate effectively with the interdisciplinary team, providing comprehensive handoffs, sharing information, and collaborating on care plans.

  1. Resident and Family Education:

  • Educate residents and their families on health conditions, medications, treatments, and self-care techniques, promoting their understanding and active participation in the care process.

  • Address residents' questions, concerns, and needs in a compassionate and informative manner.

  • Provide emotional support and guidance to residents and families, particularly during challenging situations or transitions.

  1. Infection Control and Safety:

  • Follow infection control protocols, including hand hygiene, personal protective equipment (PPE) usage, and proper handling and disposal of hazardous materials.

  • Maintain a safe and clean environment, identifying and reporting any safety hazards or equipment malfunctions.

  • Participate in infection control initiatives and quality improvement activities to enhance resident safety and well-being.

  • Stay updated on nursing best practices, evidence-based guidelines, and industry standards through continuous learning and professional development opportunities.

Qualifications and Skills:

  • Graduation from an accredited nursing program and current and valid RN license in the state where employed.

  • Knowledge of nursing principles, practices, and standards in a long-term care setting.

  • Strong assessment and critical-thinking skills, with the ability to make sound clinical judgments.

  • Proficiency in administering medications and performing nursing procedures.

  • Excellent communication and interpersonal skills, with the ability to establish rapport with residents, families, and the interdisciplinary team.
